• We suggest that you enter both spouses names in the primary contact (e.g., Bill or Angie Mullen)

• The secondary contact should be a close friend or relative - not a spouse. The reason is because both spouses generally go on vacation together or at other times may be together and cannot be reached. A close friend or relative that always knows how to contact you or one that could pick up your pet for you would be a much better choice for a secondary contact.

• The veterinarian/implanter information could be used as a third contact if you prefer.
